School entry immunisation certificates
A School Entry Immunisation Certificate is required by primary schools before your child’s entry to school. These certificates are now generated from the Australian Childhood Immunisation Registry, following completion of the four-year-old immunisations. The Australian Childhood Immunisation Registry can be contacted on 1800 653 809 or go to the Medicare Australia website.
